The data demonstrating the disproportionately younger average mortality rates of those who experience serious mental illness has triggered a nationwide services redevelopment initiative among Community Mental Health Centers. At the center of this services change is the integration of primary and behavioral health, which is critical to the early identification and intervention for chronic diseases like diabetes, COPD, and obesity.

HOPE is an education based disease self-management support program which is designed to help consumers learn to better self-manage their illness (behavioral and physical). The staff at HOPE are uniquely trained to lead consumers through Illness Management and Recovery (IMR) and Whole Health Action Management (WHAM), which are both curriculum based programs designed to increase treatment adherence and improve long term health outcomes.
