Notice of Breach Involving Protected Health Information
This notice is intended to inform you of a privacy incident that may have involved some of your protected health information. Protecting the privacy and security of the information entrusted to us is one of our highest priorities, and we regret any concern or inconvenience this incident may cause.
What Happened?
On July 1, 2026, we learned that an unoccupied former office location in Indianapolis, Indiana, had been unlawfully entered by unknown individuals. An investigation determined that unauthorized individuals accessed the building on multiple occasions between approximately June 20, 2026, and June 29, 2026. During the incident, storage areas, filing cabinets, and desk drawers were disturbed, and paper records were present within the building. Upon discovery, the building was secured, law enforcement was notified, and potentially affected records were removed to a secure location for review.
What Information Was Involved?
Our review determined that records associated with you were present within the affected materials. The information potentially involved may have included one or more of the following:
- Name
- Address
- Date of birth
- Medical record number
- Dates of service
- Diagnosis and treatment information
- Care plans or assessment information
- Insurance information
- Social Security number
- Financial information
At this time, we have no evidence that your information has been misused. However, because unauthorized individuals had access to the area where records were stored, we cannot rule out the possibility that information may have been viewed or obtained by an unauthorized person.
What We Are Doing
Immediately after discovering the incident, we secured the building, notified law enforcement, recovered and relocated records to a secure facility, and launched a comprehensive investigation. We also implemented additional security measures, including enhanced physical security controls and monitoring at the location. We continue to evaluate safeguards to help prevent a similar incident from occurring in the future.
What You Can Do
Although we are unaware of any misuse of your information, we encourage you to remain vigilant by:
- Reviewing statements you receive from your healthcare providers and health insurer.
- Monitoring account statements and explanations of benefits for unauthorized activity.
- Reporting any suspicious activity or suspected identity theft to the appropriate authorities.
- Placing a fraud alert or credit freeze on your credit file if you believe it is appropriate.
