Accessing Behavioral & Mental Health Services

Adult & Child is dedicated to ensuring that all members of our community have quick and easy access to the care they need. To become an established patient/client for behavioral and mental health services, individuals must first connect with our Open Access team to complete the New Client Process. Please note that Crisis Services are available to those who have an immediate need for care.

New Client Process

  1. Registration – Individuals will be asked to provide basic demographic information such as name, phone number, address, and insurance information.
  2. Intake – To better understand your needs, our team will ask a series of questions about your symptoms and health goals. A&C does not provide medication-only services – all behavioral/mental health clients must agree to therapy to receive prescriptions for medication. Please note that a photo ID is required to complete this step.
  3. Schedule Appointment for Services – After registration and intake have been completed, our team will schedule your first appointment with an A&C provider.

Open Access Walk-In Hours

During Walk-In Assessment Hours, individuals will be seen on a first-come, first-served basis to complete registration and intake. It’s best to arrive as early as possible with your photo ID and insurance information ready to secure a same-day intake appointment. If all time slots for same-day intake appointments are filled upon your arrival, A&C will complete registration and schedule a follow-up appointment to complete intake within 48 business hours of your walk-in visit.

  • 8320 Madison Ave, Side B | Monday – Friday, 9am – 2pm
  • Garfield Park Clinic, 234 East Southern Ave | Mondays, 9am-2pm

Scheduled Intake Appointments

Individuals also have the option to call our main line at (877) 882-5122 and choose Option 7 to be directed to an A&C representative who will assist with registration and scheduling an initial intake appointment. Please note that the 48-hour guarantee for intake appointment is available ONLY to those who utilize Open Access Walk-In Hours, and that a valid photo ID is required for intake appointments.

Insurance and Cost

Individuals do not need to have insurance to receive care or participate in services. Adult & Child uses a Sliding Fee Scale (fees are based on household income) and accepts insurance from most major carriers.
